Service Contract

This agreement, made between Mrs. Myrna Santiago McPhee of Sacred Weddings Specially 4 You (known in this contract as “the officiant”) and  ________________ and __________________( known in this contract as “the couple” is for the purpose of retaining a Minister/Notary Public/Officiant to perform a wedding ceremony on the couple’s behalf.

Ceremony:

Mrs. Myrna Santiago McPhee, being duly authorized by law to conduct marriages according to the laws of this state, will on the ____________ day of __________, 20_____ at _______o’clock in the city of _________________________, Florida officiate the marriage of _____________________________________ and ____________________________________.

• If the Minister/Notary/Officiant is unfamiliar with the chosen location of the ceremony, the couple agrees to supply detailed directions to location at least 5 days prior to the set day. Information maybe provided by email or sent in written form via postal service.
• A non- refundable deposit of $ 100.00 is due to the Minister/Officiant upon signing of this agreement to hold the above date and time for the couple; and to release sample ceremonies and vows for the couple’s review. Deposit will be paid by Paypal.com Id. sacredweddings@aol.com
• The balance of $ _______________ will be due on _____________at the rehearsal on the day of the ceremony; in the form of cash to be presented to the Minister/Officiant in an envelope along with the marriage license upon the arrival to the ceremony site. Failure to pay the above stated fees in full when is due, will release the Minister/Officiant from furnishing any services to the couple under this agreement.
• The Minister/Officiant agrees to be available to for the couple to provide support and guidance during the prep and planning stages (by email or phone) until the conclusion of the actual ceremony is performed.  Likewise, the couple agrees to be available to the Minister/Officiant for whatever purpose deemed necessary for the completion of the written ceremony.
• The Minister/Officiant agrees to supply the couple with the minimum of (6) wedding ceremony samples in the style of their chosen ceremony for their review. The couple may piece together a ceremony of their liking using the samples or write their ceremony completely on their own or chose to have the Minister/Officiant write the ceremony for them and/or the couple may write just the vows if wished.  The Minister/Officiant retains the right to edit the ceremony in case of redundancy and/or inappropriate wording/style/length/positioning/etc. up to 3 revisions maybe made prior to finished version. In case that the couple is choosing to write their own ceremony or piece together their ceremony, the couple agrees to have the chosen wording to the Minister/Officiant no later than 5 days prior to the ceremony.
• The Minister/Officiant agrees to perform the ceremony in a professional and respectful manner, in accordance to the ceremony plan chosen by the couple.
• The couple agrees to acknowledge the services being provided by the Minister/Officiant, in a dignified and respectful manner.
• The couple understands that all photographs or videos taken during, and/or their ceremony by any staff are purely non-professional, and can be used as a promotional feature in our website; copies will be available upon request.
• In unlikely event that the Minister/Officiant is unable to perform the ceremony for whatever reason, the Minister/officiant will make an effort to find a qualified substitute replacement. In the event that a replacement can not be found, the Minister/officiant will reimburse ½ of any deposit paid by the couple; however the couple agrees that the refund will be the full extend of damages they are entitle to, and no further damages may be sought against the Minister/officiant or any person affiliated to/with Sacred Weddings Specially 4 You .
• This contract will lock in the Minister/officiant to the couple’s date and time of ceremony; henceforth blocking the Minister/officiant from booking another event on the same date and time. For this purpose; if for any reason the couple make decision not to use the services of the Minister/officiant after retainment by deposit, the couple must inform the Minister/officiant in writing at least 15 days prior to the ceremony date to release the couple from this contract. If the couple fails to provide such notice, the couple agrees to be responsible for ½ the balance due as stated in paragraph # 3 on this contract.
• If the wedding is cancelled lest than 5 days before the set date and time, for whatever reason, the couple agrees to be responsible for the Minister/officiant full balance due as stated in paragraph # 3 on this contract.
• Guest, additional fees may be due and payable on time of service if guest exceed the number contractually signed upon and which fees have been based. ($ 5.00 per guest).
• Mileage, the first 50 miles to ceremony locale will be free of charge, more than 5o miles trip will incur in a .40 cents per mile according to Map quest. Com.
• Lodging when is necessary due to distance or services required, a charge of $45.00 per night will be added after been discussed on initial consultation.
• Starting time-late starts of more than 25 minutes will incur a $25.00 per quarter hour, payable at time of service, beginning 30 minutes after schedule/contractual starting time.   
• This contract, once signed is considered by all within to legal and binding to the conditions set forth herein, and all shall abide by the agreed terms.
